Route one: tongkat ali, and the only real trial here

This is the strongest evidence on the label and it deserves to be set out properly rather than summarised into a sales line.

In 2021, Food & Nutrition Research published a randomised, double-blind, placebo-controlled multicentre study of a standardised aqueous root extract of Eurycoma longifolia. It recruited 105 men aged 50 to 70 whose baseline total testosterone was below 300 ng/dL, and gave them either 100 mg or 200 mg a day, or placebo, for twelve weeks.

The 200 mg group saw total testosterone rise from week four and reported improvements in fatigue and quality-of-life measures. That is a proper study on a properly defined population, and it is not a small thing in a category where most claims rest on cell cultures and tradition.

Three caveats belong with it. It used a specific standardised extract rather than generic root powder. It recruited men who were already low, which is where there is room to move. And twelve weeks is a long time to wait before the first meaningful measurement.

Tongkat ali sits second of eight in this blend, so its ceiling is 600 mg. The 200 mg dose is arithmetically possible. Whether it is present is exactly what the panel does not say.

The comparison worth making

A standalone standardised tongkat ali extract prints its milligram figure on the bottle. If that one ingredient is your reason for buying, you can have the dose the trial used, verifiable, for a price you can see before you pay.

Route two: horny goat weed, and why the dose argument is over

Horny goat weed is first in this blend, so it is the heaviest ingredient in the capsule, and it is the one most people would name as the active. The evidence points somewhere uncomfortable.

Its active compound is icariin. In 2019, Natural Product Communications published a randomised, double-blind, placebo-controlled human safety and pharmacokinetics study of orally administered icariin, testing doses from 100 mg up to 1,680 mg.

The finding was that blood levels were very low or undetectable, with nearly all samples below 1 ng/mL. A sixteenfold dose range, and the curve stayed on the floor.

Read that carefully, because it settles a question rather than advancing one. If 1,680 mg of the isolated compound does not produce measurable blood levels, then arguing about whether this blend contains 300 mg or 600 mg of the extract is arguing about the wrong variable.

It does not prove the ingredient does nothing. Local effects, unmeasured metabolites and long-term accumulation are all live possibilities. It does mean the standard story, that icariin circulates and acts on tissue, has a measurement problem sitting in front of it.

Honest notes on the remaining six

Saw palmetto. A palm berry extract, standard in prostate formulas and a curious inclusion in a product sold as male enhancement. The National Center for Complementary and Integrative Health summarises a 2023 review of 27 studies: saw palmetto alone provides little or no benefit for benign prostatic hyperplasia symptoms. Nothing links it to testosterone.

Nettle root. The root rather than the leaf, and the two are different. Memorial Sloan Kettering notes that positive clinical results are reported mainly when nettle is combined with other herbs, including a saw palmetto combination studied for nighttime urinary frequency. As a single ingredient the case is thin.

Wild yam. On the label because laboratories use its diosgenin as a starting material for making steroid hormones. That conversion happens in a factory, not in a person. Memorial Sloan Kettering states there is no evidence that the human body can convert diosgenin into progesterone.

Sarsaparilla. Contains plant sterols, which are not anabolic in humans, and is sometimes marketed on the same reasoning as wild yam. No established dose for this use.

Orchic substance. Bovine testicle tissue, included on a very old idea about eating a gland to support the same gland. The monograph is blunt: no good scientific evidence to support this use, unclear whether it provides testosterone, and not enough reliable information to know what a dose would be.

Boron. The most defensible of the six, and genuinely modest. A trace element with a plausible mechanism, a median total intake of 1.0 to 1.5 mg a day and an upper limit of 20 mg. Last in the order, so comfortably safe at any real share.

What none of this can tell you

Whether the capsule you swallow contains a useful amount of any of it. Every study above used a stated dose of a stated preparation. This label states one number for eight ingredients, so the chain from evidence to capsule breaks at the last link, and no amount of reading on this site can repair it.
